Evening School (Undergraduate & Graduate)

  • Undergraduate Application Form PDF Document
    New student information form for the Evening School. Prospective students enrolling in classes for enrichment only should complete the top half of the form; prospective students applying for admission to a degree or certificate program should complete the entire form.
  • Graduate Application Form PDF Document Microsoft Word
    Application form for graduate study toward certificates or masters degrees. Prospective graduate students should complete the entire form, including the required attachments, and submit a nonrefundable $50 application fee. Also available in MS Word format.
  • Teacher Recertification Student Information Form PDF Document Microsoft Word
    Application and registration form for students taking educator recertification courses from the "Reaching Out to Educators" brochure. Also available in MS Word format.
  • Minor Declaration Form - Non-ESM Minors PDF Document
    Evening school students in all programs EXCEPT Emergency Services Management who wish to pursue a minor should complete this form and submit to the School of Continuing Studies office.
  • Minor Declaration Form - ESM MINORS ONLY PDF Document
    Evening School students pursuing a minor through the Emergency Services Management program should complete this form and submit to the School of Continuing Studies office.
  • Vehicle Registration Form HTML Form
    Evening School students should register their vehicles with Parking Services before classes begin. Parking fees will be charged to your tuition account.
  • Osher Reentry Sholarship Fund PDF Document
    Available to undergraduate students who:
    • Have interrupted their collegiate studies for approximately 5 or more years before being admitted to SCS
    • Look foward to a significant number of years of workforce participation after graduation
    • Are between the ages of 25 and 50
    • Seek their first baccalaureate degree (and are admitted to a baccalaureate degree program in SCS)
    • Demonstrate financial need as reported on the scholarship and in the letter submitted by each student
    • Make satisfactory academic progress in SCS (maintaining a 2.0 GPA)
  • Jean H. Proffitt Scholarship PDF Document
    Available to candidates who have been accepted to the Evening School. This scholarship is administered by the School of Continuing Studies Alumni Association.
  • Need-Based Scholarships PDF Document
    Available to candidates who have been accepted to the Evening School. These scholarships are need-based. Individuals who can receive tuition reimbursement through their employer are not eligible for scholarship assistance.

  • Instructions for Preparation of a Graduate Thesis PDF Document
    Instructions and form(s) required for submission of a graduate thesis in the School of Continuing Studies. This document applies to all graduate programs that require a thesis. Additional requirements may be provided by your thesis advisor.
